The form includes the student's academic data, the number of warnings, registered, passed, and remaining credit hours, in addition to the semester and cumulative GPA, and the date of the meeting. This data provides a picture of the student's academic level and the stage they have reached in their studies, and helps identify aspects that require attention and follow-up. The form allows for documenting the causes of academic difficulty, which helps determine the factors or obstacles affecting the student's progress, and also allows for specifying mechanisms to address the difficulty and setting appropriate steps to monitor the case. The academic advisor records their recommendations and guidance regarding the student, in addition to setting the date for the subsequent follow-up, which ensures continued communication and that the procedure is not limited to a single meeting. The form is also approved by the advisor's name and signature, documenting the advisory responsibility for the follow-up. This form is an important tool in organizing the follow-up of students who face difficulties or show indicators that require advisory intervention, by clearly and organizedly collecting their academic data, documenting the causes of difficulty, remediation mechanisms, recommendations, and subsequent follow-up dates.
