The «Make-up Exam Request» form is an administrative form used to organize requests from students wishing to take a make-up exam due to their inability to attend the exam at its scheduled time. The form aims to document the student's, course's, and exam's data, as well as the reason for absence, allowing the competent academic authorities to review the request and make the appropriate decision in accordance with approved procedures. The form begins by specifying the department and semester, along with the type of requested exam, which includes two options: the monthly exam or the practical exam. It also includes a set of basic data pertaining to the student, such as the student's name and academic ID, in addition to the course and exam data, which include the course code and number, section number, exam day and date, exam time, and the course instructor's name and signature. The form includes a dedicated section to clarify the reason for the student's absence from the exam, where the student can specify the type of excuse submitted—whether a medical excuse or another excuse—while indicating whether the report or supporting document for the excuse is attached to the request. This part helps document the justifications for absence and attach relevant documents to support the review of the request and verify the reason for not attending the exam. The form also specifies the day and date the request was submitted, and allows the student to record their signature to prove the submission of the request and assume responsibility for the data and information contained therein. The form also includes an academic review stage by allocating a section for the opinion of the student's academic advisor, recording the doctor's name and signature, and determining the opinion as either approval or disapproval. After that, the request moves to Student Affairs Committee to make the final decision on the request, while providing the option of approval or disapproval. Thus, the form reflects a clear procedural sequence starting with the student submitting the makeup test request, stating the reasons for absence and attachments, followed by its review by the academic advisor, and ending with the decision of the student affairs committee. The form is considered an important means to standardize procedures for requesting makeup tests, preserve data related to the request, and organize the review and approval process, thereby contributing to the documentation of academic decisions regarding tests that the student was unable to attend on their original date.
