It also includes a set of basic data pertaining to the student, such as the student's name and academic ID, in addition to the course and exam data, which include the course code and number, section number, exam day and date, exam time, and the course instructor's name and signature. The form includes a dedicated section to clarify the reason for the student's absence from the exam, where the student can specify the type of excuse submitted—whether a medical excuse or another excuse—while indicating whether the report or supporting document for the excuse is attached to the request.
