The attachment is Reference Section Statement Form Regarding the Academic Affiliation of the Course, prepared by the University's Programs and Curricula Department, aimed at regulating and documenting the procedures for verifying the academic affiliation of courses whose title or content may appear to be related to an academic specialty belonging to a department other than the requesting department. The form is used when needed to refer to the reference academic department to verify the extent of the course's relevance to its specialty, with the course specification attached upon submission of the request, allowing the reference department to study the scientific content of the course and express an opinion regarding its academic affiliation.

At the beginning, the form includes a section dedicated to program data, which comprises a set of essential fields that help identify the applying academic entity, most notably the college name, the applying department, and the program name, in addition to specifying the reference college or department to which the request is referred. It also includes an independent section for course data, which includes the course name in Arabic and English, the number of credit hours, and the proposed course code and number according to the specialization of the reference department, if any.
