The attachment is Reference Section Statement Form Regarding the Academic Affiliation of the Course, prepared by the University's Programs and Curricula Department, aimed at regulating and documenting the procedures for verifying the academic affiliation of courses whose title or content may appear to be related to an academic specialty belonging to a department other than the requesting department. The form is used when needed to refer to the reference academic department to verify the extent of the course's relevance to its specialty, with the course specification attached upon submission of the request, allowing the reference department to study the scientific content of the course and express an opinion regarding its academic affiliation.

At the beginning, the form includes a section dedicated to program data, which comprises a set of essential fields that help identify the applying academic entity, most notably the college name, the applying department, and the program name, in addition to specifying the reference college or department to which the request is referred. It also includes an independent section for course data, which includes the course name in Arabic and English, the number of credit hours, and the proposed course code and number according to the specialization of the reference department, if any.

The model also provides a space for entering a number of courses and their related data, allowing for the documentation of more than one course when needed, while emphasizing that Course specification is a mandatory attachment Upon submitting the request.

The third part of the form is dedicated to the feedback of the reference department, where the name of the reference department is specified, and it is then allowed, after reviewing the course description and its scientific content, to determine whether or not the course falls under the scientific specialization of the department. If affiliation is established, the course name and code are documented in both Arabic and English in accordance with what is approved by the reference department, in addition to stating the appropriateness of the course description in its current form, or clarifying the modifications made to its description. The form also includes a clear option indicating that the course does not fall under the scientific specialization of the reference department.

At the end of the form, the statement is documented through the department heads' information, signature, and date for both the requesting department and the reference department, which enhances the formality of the procedure and ensures the documentation of the academic opinion issued regarding the scientific affiliation of the course.
