Course Description: This course is designed to provide students with the foundational knowledge and essential skills in pharmaceutical care, interdisciplinary collaboration, and patient safety. The course covers the fundamental concepts of pharmaceutical care practice, including identifying drug therapy problems, collecting and assessing patient data, developing individualized pharmaceutical care plans, implementing, monitoring, and documenting them. The course emphasizes effective communication with patients and healthcare team members, interdisciplinary collaboration strategies, and the application of patient safety principles. Topics include health promotion and wellness, reengineering pharmacy practice, analyzing root causes of medical errors, and the infrastructure required to support pharmaceutical care delivery. Through case discussions and practical exercises, students will learn to work collaboratively within healthcare teams to deliver patient-centered, empathetic, and ethically-grounded care.
